Serious Intestinal Problems from Benicar

The blood-pressure medicine olmesartan, marketed as Benicar, Benicar HCT, Azor, Tribenzor and generics, has been associated with damage to the intestine, causing severe diarrhea that may lead to weight loss.

Benicar is manufactured and sold by Daiichi Sankyo and Forest Laboratories to treat high blood pressure (hypertension) in adults and children.

The experts are uncertain about the mechanism by which the medicine causes the sprue-like enteropathy or disease of the intestine, which is now linked to Benicar’s side effects.

Benicar Side Effects

The Food and Drug Administration (FDA) has said it’s still unclear on why the effects of Benicar are not observed with other drugs of the same class of angiotensin II receptor blocker (ARB).

ARBs work by keeping blood vessels from narrowing and consequently lowers blood pressure and improves blood flow.

Sprue-like enteropathy is the medical term for the damage caused by mal-absorption and intestine villus atrophy induced by a drug, not food or gluten, for example.

The Onset of Symptoms

According to experts’ research, there is a long latency period before the onset of symptoms. Then, medical examinations could find inflammatory bowel disease affecting the colon (lymphocytic or collagenous colitis), which are regarded by medicine as relatively rare conditions. More precisely, Benicar affects the small intestine villi, which are tasked with food absorption.

If you take Benicar, you should call a doctor if you experience dizziness, diarrhea, nausea, vomiting, headache, difficulty breathing or swallowing, and coughing.

This medicine is not recommended for pregnant or breast-feeding women.


The Mayo Clinic Proceedings was first to publish in 2012 that Benicar may be associated with a severe form of sprue-like enteropathy.

The Mayo Clinic results came after an evaluation of “unexplained chronic diarrhea and enteropathy while taking olmesartan”, which ruled out all cases with celiac disease and showed patients improved or recovered after discontinuation of the medicine.

Then, after review by the FDA, the federal agency issued a safety communication in 2013 warning of the symptoms and ordered a change of the medicine’s label to include the severe side effects.

“The enteropathy may develop months to years after starting olmesartan, and sometimes requires hospitalization. If patients taking olmesartan develop these symptoms and no other cause is found, the drug should be discontinued, and therapy with another antihypertensive started. Discontinuation of olmesartan has resulted in clinical improvement of sprue-like enteropathy symptoms in all patients,” said the FDA in its safety announcement.


Daiichi Sankyo and Forest Laboratories, manufacturer and sellers of Benicar (olmesartan), have been the object of growing lawsuits for the serious health problems and complications linked to their drug.

Some patients reported severe weight loss and malnutrition from chronic diarrhea, nausea and vomiting. Many sued for the harm they suffered, in some cases, requiring hospitalization and long-term medical care.

Expensive medical bills, loss of income and emotional distress, have been claimed in these lawsuits, seeking to hold the pharmaceutical companies responsible for it.

Daiichi Sankyo and Forest Laboratories are liable for knowing or should have known of the risks posed by their product, and failed to warn the patients.

Pharmaceutical manufacturers should ensure the reliability of the prescription drug they sell and if not, must be held accountable for the injuries caused if their medicines are defective or dangerous.
